Well, at first I was thinking of the diurnal rhythm of glucocorticosteroids, but it seems that shouldn't be that sharp. Not quite sure, but very interested in the answer.

Well im not quite sure but its been recently said that there's NO ACTUAL RISE in evening temp in TB. All that happens is that the patient OBSERVES or feels that his temp has risen in the evening in comparison to the day as environmental temp goes down. Might sound stupid but worth giving a thought. Cant really get the source right nw but will get back to u ppl with it once im done wid my xams. Comments on this hypothesis r invited.

So by rise in evening temperature, I correlate that with night sweats. When I think night sweats, I think Tb and lymphoma.

I wonder what's the pathogenesis for night sweats in lymphoma?

  • Other neoplasm

Infections
  • Human immunodeficiency virus
  • Tuberculosis
  • Mycobacterium avium complex
  • Infectious mononucleosis
  • Fungal infections (histoplasmosis, coccidioidomycosis)
  • Lung abscess
  • Endocarditis
  • Other infection

Endocrine
  • Ovarian failure
  • Hyperthyroidism
  • Diabetes mellitus (nocturnal hypoglycemia)
  • Endocrine tumors (pheochromocytoma, carcinoid tumor)
  • Orchiectomy
Rheumatologic
  • Takayasu's arteritis
  • Temporal arteritis

Other
  • Obstructive sleep apnea
  • Gastroesophageal reflux disease
  • Chronic fatigue syndrome
  • Granulomatous disease
  • Chronic eosinophilic pneumonia

Tuberculosis is low grade continous fever (but not eveninig rise )which is not perceived by the pt,(but can be recorded) and this gets added up in the evening with the normal evening rise of temperature which is a normal phenomenon in every body,becoz of this two (TB+Normal variation) pt feels the temp rise in the evening(its logical answer,i cann't quote ref)

ya its true.. its the history that pt tell that it has fever in evening.. becaz at day time pt is involved in its routine activity and not bother about fever. while at evening they are more conscious abt fever.. thats the reason i think...
