NT-ProBNP Predicts Patent Ductus Arteriosus in Preterm Babies

Plasma level of N-terminal pro-B-type natriuretic peptide (NT-proBNP) is a practical indicator of significant patent ductus arteriosus in preterm infants, Irish researchers report in the July issue of Archives of Disease in Childhood -- Fetal and Neonatal Edition.

"Plasma NT-proBNP level is a simple biochemical and sensitive test for prediction of significant patent ductus arteriosus in preterm infants," lead investigator Dr. Ireti Farombi-Oghuvbu told Reuters Health.

"It is a useful adjunct to echocardiography," she added, particularly "in units where there is no access to repeated echocardiography."

Dr. Farombi-Oghuvbu and colleagues at Rotunda Hospital, Dublin came to this conclusion after prospectively studying 49 infants born at less than 34 weeks of gestational age and weighing less than 2 kg.

Median NT-proBNP levels on day 3 were significantly higher in the 18 infants with significant patent ductus arteriosus than in those without the condition (32,907 vs 3147 pg/mL).

Using a cut-off point of 11,395 pg/mL, on day 3 the NT-proBNP value was predictive of significant patent ductus arteriosus with 100% sensitivity and 95% specificity.

In infants who developed late-onset sepsis, however, NT-proBNP was significantly elevated at day 10. Dr. Farombi-Oghuvbu advises therefore that "NT-proBNP levels should be interpreted with caution when patent ductus arteriosus and sepsis coexist in an infant."

nice info...so wats the test called? NT-proBNP test? readers remember: normal level on day 3--less than 11395 pg/mL
